When you fill out our consultation form, we collect the information you provide, which may include your name, phone number, email address, who needs care, when you would like care to start, and any note you add. We collect this only to understand your situation and respond to your inquiry.
When you apply to work with us through our careers form, we collect your name, phone number, email address, the area you would like to work in, your availability, and any note you add. We do not ask for a Social Security number, date of birth, or references online.
Please do not include detailed medical records, diagnoses, medications, Social Security numbers, financial information, or other sensitive health information in a website form. We will talk through those details with you directly.
We do not knowingly collect information from children.
How we use your information
We use the information you provide to respond to your inquiry, schedule a consultation, arrange care if you decide to move forward, and review job applications. We do not sell your personal information.
How we share your information
We share information with the caregivers and staff involved in providing your care. Website form submissions are processed by our website hosting provider and delivered to our team by email. We may also share information if required by law, or to protect the safety of a client or caregiver.
